We do not rush evaluations. Each patient receives a full battery of standardized tests, a thorough clinical interview, and a detailed written report — not a brief screen.
Timely turnaround
We understand that your patients and their families are waiting for answers. We aim to deliver completed reports within two to three weeks of the evaluation date.
Clinician-to-clinician communication
We are available to discuss findings directly with referring providers. If a case raises questions or warrants follow-up, we welcome a phone or email consultation.

Appropriate Referral Indications
Neuropsychological Evaluation
- Cognitive complaints in adults or older adults (memory, attention, word-finding)
- Suspected or confirmed dementia (Alzheimer's, Lewy body, frontotemporal, vascular)
- Parkinson's disease with cognitive concerns
- Post-stroke cognitive evaluation
- Traumatic brain injury (TBI) — acute or chronic
- Cognitive baseline for patients at elevated dementia risk
- Differential diagnosis: depression vs. dementia
- Capacity evaluation support
Behavioral Sleep Medicine (CBT-I)
- Chronic insomnia — difficulty initiating or maintaining sleep
- Patient seeking to reduce or discontinue sleep medications
- Insomnia comorbid with anxiety, depression, or chronic pain
- Age-related sleep changes in older adults
- Hyperarousal, racing thoughts, or conditioned arousal at bedtime
Referral process
How Referrals Work
Submit a referral
Complete the referral form below or contact our office directly. Include the patient's name, date of birth, contact information, and reason for referral.
We contact the patient
Our office will reach out to the patient within two business days to schedule an intake appointment and gather any additional information.
Evaluation & report
The patient completes their evaluation. A comprehensive written report is sent to you and the patient within two to three weeks of the evaluation date.
Follow-up available
We are available for clinician-to-clinician consultation after the report is delivered. Patients may also schedule a feedback session with us directly.
Insurance & billing
Insurance & Fees
We accept most major insurance plans for neuropsychological evaluation services. Behavioral sleep medicine services may vary by plan. Please contact our office to verify coverage prior to scheduling.
